Description:
The OK button in the Enter PUK Code page is not disabled when the page is first launched even though the PUK Code and PIN textboxes have nothing in them.

Repro Steps:
1) Updated Buri to Build ID: 20140227040202
2) Have the SIM PIN enabled.
3) Launch the SIM PIN and enter an incorrect code three times in a row.
4) When the Enter PUK Code page appears, observe the state of the OK button.

Actual:
The OK button is enabled even though none of the textboxes have been filled.

Expected:
The OK button is disabled until the textboxes are filled in.
