990773 - Input app should collect some meta data (possibly more)

The input app was rewritten to be built-in instead of linking to a website in 1.3.

Unfortunately, we've lost the ability to collect the following information that we're currently collecting on the website version of the site:

* Country of purchase (so we can get the OB)
* Name of device
* Version of Firefox OS (we get this from the user agent)

We'd like the new native input app to send this information using the Input API so that we can keep segmenting the data as we were before.

Also, we'd like in the future to have users self classify what their feedback is about (into 5 or so easy categories). What is involved in changing/improving the feedback app experience.

erm, We'd also like to possibly add star rating or other things as we work on what will be most useful to the product/engineering teams without imposing too much cognitive load on users.

CCing Fabrice as I'm told that collecting this info is also being done for the Marketplace app and he may have ideas.
