2. Notice the button is a line down from the form, but should be inline.

worksforme, linux build 2001-090-14-08. At a guess, the window you have is just
not wide enough. Try closing the sidebar?

On Linux it seems to be OK.
Also it works woth profile created with 0.9.1 (from rumors).
