"Empty" revisions are ones where the user clicked Save without making any changes to a page.

This is implied by bug 997763, but I want to make it explicit. In addition to preventing users from creating empty revisions (bug 797845), existing empty revisions should be removed or at least hidden. This applies to page histories and user histories. Empty revisions should not count towards badges or towards being listed in the "contributors" bar on a page.

This is a "garbage in, garbage out" problem.  We have to solve it on a few different fronts:

1) Empty revisions should not make it into the database.
2) Existing empty revisions should not be displayed in the UI.
3) Existing empty revisions should be removed from the database.
