Currently, page actions are detected on the URL below. On open, the device's video player opens with an "Unable to play video" warning.
There should not be any page action for this page.

The issue here is that the Samsung video player app registers itself for text/html or something strange like that, causing it to show up. We made our filters stricter awhile back, so this should have died down. Does this still happen on the S5? I think I fixed some things after this that I thought would help, but its been awhile.

I don't get the helper app icon in the address bar for the tomshardware site. I checked the article in comment 0 and several other pages as well as the duplicate bug.
