On previous versions on FxOS, the SIM toolit was available under settings.

I'm running FxOS on a flame on the nightly channel and the SIM toolkit isn't available. I can't seem to find it anywhere so I assume it has been removed.

Most mobile operators in my part of the World use the toolkit to display menus to their services tied to a combination of USSD codes. One of this services is mobile payment; Teh toolkit here's almost as important as the SMS app

Do you know your firmware version? It's a known issue (bug 1033197) on the base image of kitkat, i.e. v123 and prior to it. You may need to update the base image by referring to [1].
