Description

This behaviour occurs on the 1999081008 build of Mozilla on Windows NT 4
Workstation.

Steps to reproduce:
1. Launch Mozilla
2. Point Mozilla at this bug report.
3. Follow the link to the test case
4. From the test case, follow the hyperlink at the top of the page
5. Notice the URL bar has changed, but the page is not updated
6. Now press the back button
7. The URL bar now goes back to the original URL of the test case
8. Press back a second time to go back to this bug report
9. Now press forward again to get to the test case
10. You can now press the forward button indefinitely and the page will not update

Expected behaviour:

* In step 5, if the named anchor cannot be found, and the page we're visiting is
the same as the current page, the new hyperlink should not be added to the
  URL history.
* In step 10, the forward button should be disabled.
