From bug 1166699 comment 2: The "Add to Pocket" button in Reader View follows the visibility of the main Pocket button. (ie, if you've removed the Pocket toolbar button, we don't offer the reader view add-to-pocket button). But this visibility is only checked upon entering reader view, so that UI customization after that point will not affect the visibility of the RV button. P5 based on this being somewhat edge-case.
system addon does this, actually added a generic solution for buttons to be added/removed on the reader view. fixed in bug 1215694
