Details
BMP RLE images (and I think I've seen it before on other types of images) that
can't display properly, just show the URL in the content area. Quite misleading
(Is this a HTML document with it's own URL as the text?). Perhaps a broken image
icon, or even a error page would be better.

Another case... an HTML doc served as a JPEG. URL is the display.

I'm seeing this problem a lot. Mostly with jpg. Seems like Moz'll show the image
for a split second and then change its mind.
