Android M: Handle revoked permissions more gracefully.

On Android M, with a release version of Firefox installed, you can revoke the following permissions from the app:
* Location
* Camera
* Microphone
Whenever a website requests one of these features we show a doorhanger asking if the user wants to share camera/microphone/location with the website. However even after accepting, the website will not get any location, camera picture or microphone input if the permission is revoked from the app.
We probably want to either show the user that this permission is currently revoked (and a way out) or actively ask him for this permission in this situation.
