the default action when disabling a user is for bugzilla to continue to send them emails.  this can result in bugzilla generating and sending a lot of unnecessary email.

instead of requiring admins to remember to check the 'disable bugmail' checkbox when disabling an account we should make is so disabling an account always disables bugmail.

- tie disable_mail property to is_enabled in the user object
- fix database entries in checksetup
- update the disable_mail checkbox immediately when editing a user
