User Agent: Mozilla/5.0 (Windows NT 6.1; rv:40.0) Gecko/20100101 Firefox/40.0 Build ID: 20150826023504 Steps to reproduce: Open a page at http://uchug.org/page13.html after Firefox update to version 40.0.3. Actual results: Blank page opened without a pop up box for username and password. Tried on two PC's. One with Firefox version 39.0 opened the pop up username and password window. The one with version 40.0.3 just gave a blank page without the logon pop up window. Once the PC with version 39.0 updated to 40.0.3, it no longer opened the pop up window for logon. Tried placing an exception for the url and also allow all pop up windows in Options/Content but still would not work. It also works fine with IE and Edge. Expected results: Open a pop up window to type in a user name and password.

Also tried in safe mode and still gives a blank page without login pop up box.
I guess dup of Bug 1197944 Could you try setting network.auth.allow-subresource-auth to 2 in about:config?

Hi Alice0775 White, Your suggestion fixed the issue on my PC. Hopefully, there will be a general fix for this in one of the Firefox updates. That way anyone I have given access to that page will not have an issue without having to make config changes. Thanks for the help! webmasters@ucchug.org

I no longer need a correction for this issue. The web page and logon are no longer needed and the page has been removed from the web site. Thanks, Bob

Grover, this has nothing to do with form controls. This bug was about the HTTP authentication dialog no longer coming up for a subresource on the page. Please see comment 2 and comment 4.
