Steps to reproduce:
I have set up Firefox to always restore the previous tabs when it starts. I heavily rely on this feature, keeping tabs open to remind me of things I still wanted to look at.
A while ago (not more than two months), Firefox started to sometimes forget the tabs that I had opened: I would close Firefox (or sometimes I would find it closed and be surprised, since I didn't want to close it, maybe I accidentally clicked the "X" in the corner), then I would start it again.
Actual results:
Only my homepage opened.
Expected results:
Instead, it should have opened all the tabs I had open previously.
This is a rather bad bug, since it result in data loss: Right now, I know I had three tabs open that I cared about for later, but I can only remember two of them. I can just hope the tab I can't remember was not too important...
Sorry for the vague description, I am unable to reproduce the problem. It now happened around 3 times within the last month. From the description, this sounds a lot like bug #747560.
I am using the Debian Iceweasel package.
This just happened again. I had >20 tabs open, and had to reboot the machine. They are all gone now. I'll have a fun time remembering what they were all about...
I since then switched from Iceweasel to the official Firefox builds, but just recently (with Firefox 44), this happened again.

Ralf, when it has done this, it always forgets _all_ tabs, correct?

Correct.
I have not seen this happen in the last two months (where for the last ~4 weeks, I used the Debian Firefox, and before that, the upstream one) -- but this was a rare bug to start with...
