Attempted to re-image it twice, but it got stuck while completing the re-image (first time) and then while puppetizing (second time). The IO wait times don't look too good, so maybe some diagnostics could help on this.

memtest ran 65 passes with no errors, running disk diags now.
disk diags taking 4 hours to finish when it usually takes ~90 minutes. memtest ran 65 passes with no errors so RAM is ok. opened #IAW-615-11073 for disk RMA.
