128866 - mozilla can't access ActivCard certs after a crash

after mozilla crashes and is restarted, it is unable to access certificates
stored on an activcard smartcard.

I use my cert to access my SSL/IMAP mail server but when mozilla crashes and I
restart it, the program prompts me for my username/password instead of my
smartcard passcode.  Just before it does that I can see it access the card.  it
blinks very rapidly for about a second before failing and prompting for the
username/password.

When the card is in this state, mozilla also believes that I'm not logged into
the card:
go to preferences -> Privacy & Security -> Certificates -> Manage Certificate
Devices
click on "activCard" and it will show up as "not logged in".
Clicking on "log in" prompts for the activcard passcode, but I'm still not
logged into the card.
