Steps to reproduce:
1. Create extension proxy as page https://developer.mozilla.org/en-US/Add-ons/Setting_up_extension_development_environment#Firefox_extension_proxy_file shows
The addon/extension is a traditional XUL one.
2. Restart Thunderbird
Actual results:
Nothing happens.
Firefox has switched to web based extension, but the old should still work with TB.
Expected results:
Thunderbird should install the addon

That's on a recent TB Daily, I assume?
There was another complaint about "side-loading" in bug 1389189.
Kris, did that get removed intentionally or damaged accidentally?

It was intentionally removed. We only scan directories at startup if they're in the extensions.startupScanScopes preference. Other side-loads are added later, if and when AddonManagerPrivate.getNewSideloads is called.
