It seems that when you approve a revision, the r+ mirroring is done using the patch author's account, so on the one hand if I receive an r+ I don't get bug mail about that, which might be okay I guess since there's a separate mail from Phabricator anyway. If on the other hand I approve somebody else's revision, I do get a possibly unneeded mail from Bugzilla because technically the r+ on the bug was set not by me, but by the patch author.
This should be fixed now with recent updates to BMO (bug 1482145). Please reopen if still occurring.
