ctrl-click or middle click on the link toolbar should have the same effect as
those actions within a page -- spawning a new browser window with the bookmarked
link, or opening the link in a new tab.
Which build Do you use ?
Do you mean the "site navigation bar" ?
Bug 59132 covers middle-clicking and ctrl+clicking on personal toolbar
bookmarks.  I'll assume this bug is for the site navigation bar.

Good call. It'll be a while before I can get to this (if I ever can), but it
should be possible for someone else to do it if they're vaguely familiar with
event handling in XUL. Look in linkToolbarOverlay.{xul,js} for the "commanded"
function.
