Steps to reproduce:
Open a new message window
Actual results:
To: field is open and ready to receive input
BCC: field is populated with my preset recipients
CC: field is not available and has to be triggered by clicking the CC: option with mouse.
Expected results:
Either allow CC: to be a user-settable option (not currently available in options or account settings) that always presents a field ready for input, or provide a keyboard shortcut to toggle it. Currently, every time one wants to compose an email with CC: recipients, an additional mouse step is required.

Mark Hepburn, thank you for your well-presented proposal. My thoughts exactly, and several users especially from enterprise contexts have requested the same. ux-efficiency matters!
The user-settable option to have CC always open for a certain identity is Bug 1616514. That's definitely doable, watch that space.
I'd love to have gmail's highly intuitive keyboard shortcuts for To/CC/BCC in Thunderbird (Ctrl+Shift+T/C/B); however, that's somewhat trickier as we'd need to resolve conflicts with current and future shortcuts, or think of some other way of avoiding or handling those conflicts.
Inspired by your proposal, I've filed Bug 1667692 to explore the feasibility of having those (or other) shortcuts.
