Description

(Bug 133559). 

This has been in production on b.m.o for a while now, and I think we should
check it into the main tree as an example of what you can do with custom
template and enter_bug comment templates (which I've already had to explain
once, despite it being in the Guide.) Also, this will make it much easier for me
to work on enhancements for it (e.g. bug 170464), and keep it current with any
template API changes.

Myk: does this seem reasonable?

This seems more than fine to me; I think it's a good idea.  I'd like Dave to
concur first though.

Sounds good to me.  If you don't genericize the references to Mozilla in it, you
should probably add (user-visible on the resulting page) a short comment that
this is an example page to show what you can do and the admin probably needs to
customize it for their needs if they want to use it.
