RFC 9078 envisages the possibility to react to a message with just an emoji, not a full reply. See:

Expected results:
It would be nice to collect (pointers to) reactions in the list folder, perhaps showing the icon or the count in its own column, still allowing to examine the content of the message(s), which would be collected somewhere else.
(This would be nice to have for return receipts and DSNs as well.)

The spec is careful not to give UX guidance, since it's out of scope for a protocol spec.
My personal thoughts are to have authors signal their reactions in a manner similar to the way we do it on social networking. For email, a reasonable possibility is to hit Reply or Reply all and have the header portion of the reply window include an line below Subject, with a common set of emoji choices. The line might have a title such as "Reaction" but I suspect there are better choices.
For the receiver of such a message, there could be a similar enhancement when the message is displayed. But there also might be an indicator in the frame showing the list of messages in a folder. An additional column for showing a reaction contained in the listed message.
