Transfer campaign-form.herokuapp.com to Mozilla Heroku account

Similar to proton-form (bug 1701706), we would like to transfer campaign-form.herokuapp.com to the Mozilla org. This time it's a generic implementation and we won't need to do any more migrations for future campaign forms.
Additionally we won't need the proton-form anymore, and therefore https://dashboard.heroku.com/apps/proton-form can be deleted.
To transfer it, please change my existing role to "member" and then once transferred we can downgrade my access to "collaborator" again. However I will need full access to the "campaign-form" app.
Konstantina, can you please approve this from your side?

Thanks for the fast help, Jon. I have transferred the app. Feel free to downgrade me to collaborator again. Additionally https://dashboard.heroku.com/apps/proton-form can be deleted, as we won't be needing this anymore.
