Unfortunately you cannot use the short form to make a temporary collection permanent. You need to use the full form because it asks the necessary additional questions.

I'm unsure what you mean, the linked previous data collection was already about something permanent: https://bugzilla.mozilla.org/show_bug.cgi?id=1753424#c9

Yes. It's in CrashAnnotations.yaml.
- Is there a control mechanism that allows the user to turn the data collection on and off?
Yes. Crash ping data is covered by Telemetry and crash report data is covered by crash reporting settings.
- If the request is for permanent data collection, is there someone who will monitor the data over time?
Yes. alissy@mozilla.com
- Using the category system of data types on the Mozilla wiki, what collection type of data do the requested measurements fall under?
Category 1.
- Is the data collection request for default-on or default-off?
It's default-on.

No.

Yes.
