Since bug 174049 got fixed, smooth scrolling is now built into Phoenix, but not
enabled by default.
We should set general.smoothScroll to true by default in all.js.
No UI is needed for this.
There is a similar bug for Mozilla: bug 198964.

IMHO this should be held back until Bug 199107 and Bug 199024 are fixed.

After installing 1.4a, the pref was not added, I had to do it manually. Should
it have been?

lohphat: The answer is yes. Please, read the release notes (in this case "What's
New in Mozilla 1.4a" section).
You may use the about:cpnfig interface frequently, but I do not. It's
non-obvious that the interface can be used to add prefs. Ijust headed to the
prefs.js file directly.
Related question: Shouldn't pop-up contect menus replicate actions available
from the pull-downs? The right-clock actions of adding prefs in the
about:config window are not found in the edit pulldown menu.
