215994 - "Remember password?" dialog appears twice

When you go to a site that requires a login, you typically type in your user
name and password, and hit "Login".  The "Confirmation" dialog box (with three
buttons; "Yes", "Never for this site", and "No") shows up.  The problem is, it
shows up TWICE.

Reproducible: Always

Steps to Reproduce:
1. Remove your profile folder to start anew.
2. Go to http://forums.mozillazine.org
3. Hit "Login"
4. Type in your username and password, and then hit "Login"

Actual Results:  
The "confirmation" dialog box shows up twice.

Expected Results:  
The "confirmation" dialog box should show up only once.

Confirmed using the an unofficial 08/12 Windows build from
http://forums.mozillazine.org/viewtopic.php?t=19698.  This happens whether I say
"yes" or "no" to the first dialog.  (If I say "yes", I also get a
set-master-password dialog before the second "remember password?" dialog.)

I would like to have it reopened.  I still see the problem with the 2003-08-14
build for Linux.

Please re-open. I see the problem with Mozilla/5.0 (Windows; U; Windows NT 5.0;
en-US; rv:1.5b) Gecko/20030814 Mozilla Firebird/0.6.1+ (which is Sipaqs
un-official build). There is definitely something wacky with the Password
Manager. I am using existing profile.

Can anyone replicate this with a clean profile, not an existing one?

Whenever I submit a bug, I always test it with a clean profile.

To reproduce the problem, please press "Never for this site" when the "remember
password" dialog box shows up for the first time.  Then the same dialog box
shows up again (for the second time).  Please also test it with the Linux build.
