I see hundreds of Proxy bug reports, but haven't been able to find this one:

I have Firebird 0.6.1 set as my default browser on my Windows 2000 workstation.
 If I click on a link in an Outlook e-mail message or type the URL into the
address bar built into the Windows taskbar, Firebird will load and immediately
say it can't find the server.  Click that link again, or type that address again
in the taskbar or in Firebird itself, it will come up immediately as long as one
window of Firebird is loaded.

I'm using an HTTP proxy server configured using a pac script.  

Reproducible: Always

Steps to Reproduce:
1.  Make sure Firebird is not running
2.  Make sure Firebird is set as default browser
3.  Turn on Address toolbar in Windows 2000 taskbar (if not already showing)
4.  Enter a valid URL into the Address bar
--> symptom:  Firebird claims it can't find the host
5.  Click on the address bar (the URL you typed should still be there) and press
enter without changing anything
--> symptom:  Firebird loads the page immediately without missing a beat.

Actual Results:  
Firebird doesn't load URL first time through, almost as if it's not aware of the
proxy script.  Will load an intranet (no proxy required) page just fine.

Expected Results:  
Should detect proxy settings during initial program launch and go straight to
the URL.
OS: Windows XP → Windows 2000
Update:  A proxy server is not required to reproduce this problem.  I just did
the same thing on a Windows XP box with no proxy settings.

Marc (reporter) - can you reproduce this with a recent version (e.g. Firefox
0.8)?  If so, please comment again with details.  If not, this bug can be
resolved WORKSFORME. thanks.

Verified bug still exists in Firefox 0.8.  Didn't do it at first.  No changes
made to browser to trigger.  Maybe it's something that happens on its own after
a while???

When proxy server and port numbers are entered manually, the problem goes 
away.  Problem only persists in Firefox 1.0 RC1 when URL to proxy script 
(script is enclosed).

Can also be reproduced by clicking on a URL in Outlook 2000 or Outlook XP with 
Firefox 1.0 RC1 selected as the default browser.

This sounds like it might be the same as Bug 100022.

I think it is.  I agree with the other posters' comments... it's not obvious
because bug 100022 does not come up on a search for keyword "proxy".
