When entering data on www.producttestpanel.com/address.php, I receieve a debug
error and it states that another instance of Visual studio 2003 is running.
Mozilla then crashes. This also has happened on a website for Mr. Clean, I use
the form manager to fill in the form.  

Reproducible: Always
Steps to Reproduce:
1.go to site www.producttestpanel.com/address.php
2. enter data w/ form manager
3.step down to agreement.

Actual Results:  
Mozilla crashes

Expected Results:  
accept data, and go to next page

visual studio 2003 debugger comes up
since you're running Mozilla in a debugger, can you attach a stacktrace for this
crash ? Otherwise, run mozilla.org releases and post Talkback ID for this crash.

I registered a fake account and looked at the surveys. Each survey started with
a question, and Radio Buttons YES and NO. When I clicked 'NO', Forms Manager
came up and asked me if I wanted to remeber my decision. After answering, next
survey started. When I clicked 'Yes', nothing happened. Then clicking 'No',
Forms Manager came up, I answered, and next survey started.
The difference is that I have Visual Basic.net 2003 installed on my machine. 
This problem also happend on another website that was a survey.

The system posted a notification when it crashed.
can you run "mozilla\components\talkback.exe" and mention the Talkback ID
(generally in the format TB123456X) associated to your crash ?

Since I am using 1.8a2 I get nightly builds. The last build deleted the
incidents from my computer.

 I went into the web page and reinacted the events and generated a Talkback ID
for this problem TB303011G. The web page was
