Clarify documentation on the use, structure, and rules of the Bugzilla::DB package

Details
The Bugzilla::DB package has a lot of "rules" to it that are right now only in
my mind, although they are probably known by Tomas and a few others.
Bugzilla::DB::Schema is also a similar thing.
We probably need two things:
(1) A "hacking Bugzilla::DB guide" (can be in the POD docs)
(2) A thorough review of the API documentation for all of Bugzilla::DB

Oh, we also need a "How Database Upgrades Work" thing, probably based on bug
285111 comment 5.

Documentation stuff is not enhancement (in the sense that it's too late for enh bugs on branches).

No, this is actually an enhancement. It doesn't change severity just because we're frozen.
