The cookie exception dialog does not close with the OK button if the user clicks
"Remove" when a blank entry is selected.

Reproducible: Always

Steps to Reproduce:
1. Open the cookie exception list.
2. Remove an entry by selecting it and choosing "Remove site".
3. Click the empty space below the last entry so it is selected. It will already
be selected if you removed the last entry in the list.
4. Click "Remove site".
5. Click "OK" to dismiss the dialog.
Actual Results:  
OK button does not work. Dialog must be closed with the cancel button or window
manager's close button.

Expected Results:  
OK button should close the dialog.

It looks like the entry is not properly removed from the list, leaving an empty
list entry at the end.
The pref dialog has been rewritten, this no longer happens.
