I'm seeing this too. Fx 1.5.0.4 and 1.5.0.5 on Win98.
It appears that Fx is opening two copies of the "Enter Master Password" dialog in exactly the same location, one on top of the other. (I happened to be looking at something else in DOM Inspector when this occured, and the master password dialog showed up twice in the DOM Inspector window list).
To reproduce.
0. Tell Firefox to use a master password in general.
1. Create a free account on this site. http://www.gamepark.eu/
2. When you login to the site, say "Yes" when Fx asks if you want to remember password for this site.
3. Restart Fx.
4. Enter username on http://www.gamepark.eu/, then tab to the password field.
5. Fx asks for master password. This is where the error occurs.
It may happen for other sites as well but this is where I've seen it.
I'm seeing something similar.. if you want to enter a username and password in a dialog WITHOUT typing in the master password (like a guest would, say, log into gmail on my computer which I have saved a username/password for), the guest would click "CANCEL" on the master password dialog. Then type the first letter of their username. The dialog sees activity and asks for the master password. The dialog is CANCELed. The second letter of the username is typed. The Master Password dialog pops up.. etc etc.. This didn't used to be this way and this issue should be resolved.
Exists in Fx 2.0.0.1 on XP (seen back in 1.5.x.x on XP too)
Similar to mcdavis941's report, but I think I can clarify a little further:
The Software Security Device master password required dialog pops up twice (one over other) if there is more than one username+password stored by it for the current URL. Both pop up as soon as non-blank username field loses focus.
If there is only one username+password stored for the URL a single dialog pops up normally immediately on landing at the page.
Interestingly (in the first case) if both dialog boxes are cancelled, a third pops up! (I observe this is at URL with 3 or 4 stored username+passwords, but I haven't checked how/if this relates to quantity)
