Late changes on en-US Help
Correct if I'm wrong, but this patch didn't land.
Note, the firebird-toc part of this patch was landed by Steffen as part of bug 352968.
I'd do a separate evaluation of the menu_reference.xhtml part, if I get a patch.

Closing this bug as the firebird-toc.rdf part has been fixed meanwhile by Steffen. So the rest of this patch doesn't worth to be taken as it was just a polishing transcription of the en-US help late changes.
