The error message you get when an attachment upload fails doesn't stand out enough to be noticed alongside the "new bug created" message which is directly under it. It also doesn't tell you *why* your attachment failed, only that it did. We should probably boldface the error and make it red instead of green, and try to get the actual error from the attachment upload included in it.
It should be easier to achieve after bug 364177 lands as you could then use the ERROR_MODE_WARN mode.
