If I browse http://www.sitemeter.com/ while I am in Norway (does not happen in Germany) green card ads show up that link to:

Some pages at TopWebComics.com generate an iframe containing a flash file. To reproduce, visit http://topwebcomics.com/ and click on any of the "Vote for..." links.

The resulting page contains these calls to remote JS files, which create the iframe; the live page points to these files:

http://ipoint.targetpoint.com/resources/js/getchanel.js
http://ipoint.targetpoint.com/resources/js/imagepoint.js

I had to save (HTML complete) the live page locally to get the URI of the flash file (locally in tag.html, if you're curious), for example:

Ran across http://www.gamefaqs.com/portable/gbadvance/ while testing another bug; 

iframe src=http://media.nyadmcncserve-05y06a.com/iframe?spacedesc= (with flash ad whose provenance I can't discover)

We block flash from "media.nyadmcncserve" already, but not iframes.

Getting Flash ads from adinterax.cnet.com.edgesuite.net when I view topic threads at Chowhound.com. Sample recipe:

1. Go to http://www.chowhound.com/
2. Click on a topic under "Latest Posts"

Results: Flash ad appears in a square-shaped region on the right side. For example:
