Example: when there is an email with many addresses in the "To:" list, we see a + button to the left of "To:" which when clicked will expand the address list. This needs to be keyboard navigable/operable.

I agree that the field should be operable.  I think making the + node a separate focus point would be a poor solution; far better to simply add a new action to fields that have such a node (including a new entry in the popup menu to trigger it).

Tentatively morphing to reflect new realities in message header: [+] twisty was removed, we now have _3 more_ link and it's even keyboard-focusable (as it should; tab out from last visible address and then press Enter to test), but the focus doesn't show because focus ring is missing.
