I'm seeing this on a fairly regular (daily) basis but don't have a recipe to force it to happen..
Basically, I will have 3-4 tabs open, and I'll switch from one to another - and the URL up in the location bar will fail to switch. Once it gets into this state, it's pretty harsh - I can even hit control-T for a *new*, *empty* tab, and the stuck URL will display.

Thanks for the cluebat - that hint was what I needed to go back and find the *real* problem - the offender was the 'Verisign Green Bar Extension' (https://addons.mozilla.org/en-US/firefox/addon/4828), which has apparently hosed others as well - https://addons.mozilla.org/en-US/firefox/discussions/comments.php?DiscussionID=6266 states that when bug #398020 landed, it broke.
So I guess this should be redirected to wherever "Bug #398020 broke my extension" whinging goes...
In the meantime, I was able to just disable that extension and things are working again...
