Printing headers and footers under MacOS X 10.3.9 does not work

The File -> Page SetUP menu does not allow selection of headers and footers.
However, you can get them to print for only the current document using the Print dialogue.
The curious thing is that when I do an ABOUT:CONFIG, I see the header and footer settings there from the last document that I printed as above.
However, they do not continue to be used with subsequent prints.
Reproducible: Always
Steps to Reproduce:
1. File -> Page SetUp has no header or footer options
2. Using the Print dialogue, you can set them for the current document
3. Headers and footers do not continue to print on subsequent documents
4. Curiously, ABOUT:CONFIG shows that the header and footer setting are there, but they are not used for some reason.
Actual Results:
Header and footers only print once and do not stay in effect/
Expected Results:
I expected to see the header and footer selections on the File -> Page SetUp display. And, once set, I expected to see them used.

This WFM on 10.3.9 with 2.0.0.11.
First off, headers/footers have never been configured in Page Setup on Mac OS X.
When I enable/disable/change a header or footer in the "Firefox" page of the Print dialog, the document prints with that configuration, and the configuration persists in the next document I print.

They didn't stick in my case, they always are reset to defaults the next time I get to the Print dialogue. Also the combined action of this bug and of bug #412777 was lethal to me, since I had absolutely no way to set headers and footers in Firefox! I had already downloaded and installed Opera when I parallelly found these bug reports and understood what was happening. Anyway, please fix at least one of the two bugs... and I think that there should be a Headers/Footers tab in Page Setup under Mac OS X as well, that's where all user documentation and friendly help available on the Web points users to, it's really confusing not to find it.
I am Mozilla/5.0 (Macintosh; U; PPC Mac OS X Mach-O; en-US; rv:1.8.1.3) Gecko/0000000000 Firefox/2.0.0.3, the G4-optimized build (Bon Echo) (so I can't update).

There's no "Margins & Headers/Footers tab" in the Mac version of Firefox.
Tested on Firefox 3 on Mac OS X 10.5.5.
