In the "Tabs" sub-menu of the "Tools/Options" menu, even if I select "a new tab" under "New pages should be opened in:" all new pages are always opening in the same tab.

Reproducible: Always

Steps to Reproduce:
1.Select "Options" under the "Tools" menu
2.Select "Tabs"
3.Select "a new tab"
4.Click "OK"
5.Open a new page
Actual Results:  
All new pages always open in the same tab

Expected Results:  
New tab opens each time a new page is opened
Type about:config in the location bar and press enter. Confirm that you know what you are doing and search for the pref browser.link.open_newwindow , right click and choose Reset.
If this doesn't help it has probably another cause, and a new profile will probably fix this. For a more refined solution (I don't know enough details) you could visit the MozillaZine forums: http://forums.mozillazine.org/viewforum.php?f=38
Actually, that option doesn't quite do what it promises to (see bug 356830): it only applies to links that would have opened in new windows otherwise - which is only a minority of all the links you'll ever encounter.
