Seen while testing Mozilla/5.0 (Macintosh; U; Intel Mac OS X 10.5; en-US; rv:1.9) Gecko/2008051202 Firefox/3.0.

STR:
1. Open Firefox in Safe Mode (hold Option Key while FF starts up)
2. Check the box that says "Disable All Extensions" and then "Continue in Safe Mode"
3. Observe attached screenshot.

Expected: All extension would be disabled and the "Enable" button would not be available.
Actual: The extensions appear to be enabled, although they are not actually installed.

I can reproduce this with the new add-on manager assuming that you mean to click "Make Changes and Restart" in step 2.
I can no longer reproduce this problem.
