I'd suggest doing this:

it's related to the design for the client-cert request dialog, given that, in both cases, the site is asking you to identify yourself.  (http://people.mozilla.com/~madhava/files/mobile/2008-11-18/certificate.png)

I think we can just use the new prompt service API (bug 489423) for this dialog. The toggle would be slightly different, but basically the same as the userpass.png design

Bug 514196 landed and made the auth prompts look like:

Bug 514208 will cover making these prompts look better

We could add unique styling to this particular dialog. The image for example.
