I definitely think it's a caching issue. I just don't know where.
If you log in and visit any of those URLs, it works (the inproduct style is always applied to Firefox Help, which is why you can do this while logged in). Locally, where I've been working with memcache turned off, this works. On stage, it doesn't.
The only thing controlling whether or not the survey shows up is the page, locale, and preferences. The page and locale aren't changing, so it has to be the preferences.

All URLs from comment 4 are now displaying the survey for me even when logged out, so it was definitely a caching issue.
I'm calling this fixed -- nothing more to test, here.
