This bug is essentially identical to what I reported in bug 47247 (which was
determined to be a dupe of bug 25684). On the advice of Asa, I am filing a new bug.
Basically I was able to get locked out of Mozilla by switching focus away from
Mozilla as the Password Manager was prompting me for what password to use at
Mozillazine. Focus remained on the main Mozilla window even when the password
dialog box was brought to the front of it. Could not click on any buttons, or
even right-click on Mozilla on the Taskbar and close it. Had to end-task using
Task Manager.
I will include a screenshot.

Please give step-by-step instructions for reproducing this. For example:
1. Go to http://mozillazine.org
2. Click on the "x responses" line following any article
and then what did you do to bring up the dialog shown in your screenshot?
I'll try and help, but realize this is not 100% reproducible. You have to
switch windows (apps) at just the right moment.
On a previous visit to Mozillazine I had responded to a post. Password Manager
asked me if I wanted to save my username & password, which I did. Mozillazine &
PM don't work well together though as on your next visit to a thread (not the
home page... you have to pull up a thread where you get a form to add a reply)
it doesn't get that it's the same password, so you get prompted for which one to
use on this site (that box you see in the screenshot).
I was switching away from Mozilla itself to another app right after I clicked on
a thread. I was switching back to Mozilla and caught it at the wrong moment to
cause it to lock up the way I described earlier. I have no way to know whether
it was the switching away from or switching back to Mozilla that triggered the
lockup, but other than the nature of the box that is popping up, this is
otherwise identical in nature (as far as I can tell) to bug 47247.
This never happens if you just let it sit on Mozilla and pop up normally. You
must be switching apps, and at the right moment.
Hope this helps.

OK, that explanation helps. I didn't recognize the pop-up box shown in the
attachment but from your explaination I now realize that it is the dialog that
password manager puts up when you go to a site for which you previously saved
more than one username/password combinations. In this case you didn't really
save a username but password manager didn't know that -- it looked for the first
non-password field on the form and assumed it was the username. The field
happened to be the title of the message that you were responding to.
None of this explains the bug yet. But it does clarify what it was that you
were doing and what the popup box was that you were seeing.
I would imagine that there is nothing unique about mozillazine and that visiting
any site for which you stored multiple username/password pairs would demonstrate
it. I'll try to see if I can reproduce it by submitting the dummy form at
http://people.netscape.com/morse/password.htm
which is the form I use for debugging password-manager problems.

Am unable to reproduce. I have experienced this sort of problem in the past on
other dialogs having nothing to do with password manager. So if there still is
a problem here, it's probably a general dialog problem having nothing to do with
either password manager (single signon) for forms manager (autofill).

I agree that it has nothing to do with the Password Manager... :) I was advised
to file a separate bug though. Tried to make it clear at the beginning of this
bug that I thought that it was the same as bug 27427.
This has happened to me twice today already. One was another Password Manager
dialog... Mozilla opened up an additional window for a site that prompted for
username and password. After entering them and clicking OK, the PM dialog
popped up offering to save these for me. I think I might have clicked on the
previous window (one with my user/pass) right as this was happening. Sure
enough, same lockup as before.
The other time today was during a new action: the Save As... dialog box when
saving a link as a file. I can't remember the exact series of clicks that
locked it up, but this time it was the Save As box that was in front but
couldn't be made active. Exact same symptoms otherwise.

This just happened to me again. Not with PSM this time, but a much more simple
example where I clicked on a link in Mozilla, switched to a Window, and then
switched back to discover that at some point after I clicked the link a dialog
box had popped up about not being able to connect to the site. However, focus
could not be switched to the dialog, even though it was in front of the Mozilla
window. Same behavior as before, and I had to end-task on Mozilla.

And it happened to me yesterday as well. I was trying to install the PSM
module from the i-planet site. While it was installing, I switched windows.
When I switched back, I could not get focus in the download window. I couldn't
cancel in that window or close it, although I had it as the front window. I
finally had to close the browser and restart it.
Changing from UNCONFIRMED to NEW.

Thank god someone has finally seen this happen besides me :)
Morse: you said you closed the browser. Did you do this from within Mozilla
itself, or via End-Task from the/a task manager? I was not able to get Mozilla
to respond enough to have it close itself.

Don't recall but I think I might have had to use the task manager.

Sounds like a duplicate of bug 39841 "modal window lost focus, now mozilla
essentially hung".

This happened for me also. It wasn't a focus problem (I think). The password
manager dialog box refuses to accept any clicks even though it seems to have the
focus. It is impossible to do anything on any page that needs a password now.
This is with the 0.7 release on Windows 2000.
And I was just about to make the switch from Netscape 4.76....
