This appears to be the same as or very similar to bug 21611 which is marked
"verified fixed".
On Mozilla M18 build (2000101015 Linux) go to any page which refreshes via
http-equiv refresh and wait through several refresh periods. The URL above
refreshes every 45 seconds, so is a good one. Click on the drop-down browser
back history button and see that the page has added itself to the history every
time it refreshes.
This might not be quite so bad if each entry in the history were actually a
snapshot at the time that the refresh occurred, but that is not the case --
selecting any of the entries simply reloads the page at the current time.
If this was truly fixed back in April, it appears to have come back. It also
occurs in the M17 build.

I'm definitely seeing this here. I'm not sure whether it is more proper to close
this and re-open the original bug, or just leave this one (since it seems clear
that they are duplicates) but either way it ought to be fixed.

I've been seeing this for quite a while. Look at my last attachment to bug
44469 for some cgi scripts that can easily demonstrate this.
