In some scenarios (Bug 599034) you can have orphan tabs be hidden behind groups. See the second half of the screencast http://screencast.com/t/ZTBmMzY1 We should be able to see all tabs at once.
Looks like what happened is that the tab (which had previously been part of the group) correctly loaded its location inside the group, but failed to load the fact that it should be a child of that group.
