This problem has something to do with tab groups. Often, a few open tabs will disappear from the tab bar, and when I check the Tab Groups page, I see the aforementioned tabs in their own tab group or with no group. I'm not sure what causes this, as it happens randomly.

This has happened to me under Vista x64, Ubuntu x64 Lucid and Maverick, and has been happening since the tab group feature has been introduced. The optimal solution, I suppose, would be to make a setting to disable this feature.

Reproducible: Sometimes

Steps to Reproduce:
1. Browse with Firefox 4.
2. Eventually, tabs start disappearing from tab bar.
3. Viewing the Tab Groups page shows them outside of the main tab group.

Effect:
1. Open the Tab Groups page.
2. Drag a tab outside of its group.
3. Return to the main Firefox 4 view.
4. The tab has disappeared from the tab bar.
Actual Results:  
Tabs are removed from the tab bar and placed in separate groups only shown when viewing the Tab Groups page.

Expected Results:  
Not removing the tabs from the tab bar.

I get this Behavior/Symptom occasionally when i want to use the "Undo Close Tab" Function from the Tab Context Menu. Quite annoying this is ... .

So regarding your Step (1) try to find a Pattern of Actions getting to this Behavior.

@juan: this is not happening when going to private browsing, but rather, it happens at random times (I am currently trying to figure out exactly what causes it to happen). Therefore, I would not say that this is a duplicate.

This might happen with PB interactions as well as update/restart interactions. I think there is a but with the latter case, but I can't find it off the top of my head or from my awesome bar :)

Huh, that seems to be the most likely case, as I can confirm that restarting Firefox does indeed trigger this bug. I do recall having this problem usually after restarts, making new tabs open their own group and sometimes causing closed tabs to instead disjoin from their group.
