616490 - Update create-guided.html.tmpl to latest b.m.o. version

With the exception of the banner at the top in the standard distribution, create-guided.{html|txt}.tmpl have always been Mozilla-specific, and included in Bugzilla as an example demonstration of bug entry templates.

The copy on b.m.o. has diverged from the one in Bugzilla 4.0 and on trunk. We should update the copy in the Bugzilla distribution to match.

This contains code that is ultimately very specific to the bmo customizations. I'd like to at least ship a template that will compile.

and which passes tests. There are several unfiltered fields.

Seeing the "Follow bug" button for open tickets next to each search result in the guided form, this is something that Wikimedia design team also asked me for. I'm fine with configuring strings and values for our needs, but I'd really appreciate functionality to be ported from bmo to upstream.

The bug is titled correctly. Read the patch or read bug 920681 comment 1.
