This problem starting occurring for me in the FF4 beta. Firefox will crash within 5-10 seconds of initial loading, with no warning, error, or dialogue. The Crash Reporter has only popped up once in about 100 or so crashes (enabling this bug report, fortunately).
Firefox will crash even if started in Safe Mode. Eventually, after somewhere from 3-10+ startup attempts, Firefox will load and will work normally until closed and restarted, where the process starts all over again.
I have completely uninstalled Firefox, deleted all profile directories, manually removed any Firefox or plugin related entries from the registry, etc. and then re-installed from scratch, but the problem still persists. Judging by the comments that appear on my crash report, this problem is not unique to my system, but may not be widespread either.

Steps to Reproduce:
1. Load Firefox
2. Wait 5-10 seconds
3. Watch Firefox disappear with no error, warning, or dialogue box of any sort
Actual Results:
Firefox will crash repeatedly until eventually (after 5-10 tries) it loads and works normally.
Expected Results:
Loaded and worked normally the first time.
I have disabled all themes, plugins, etc. and this problem still occurs.

Also, another user with this error reported that quickly switching any page other than the default home page before Firefox died would prevent the problem. This can be difficult to do since you generally only have a second or two before the entire application disappears, but in testing this with a site button on the bookmark bar, it seems to be accurate. Don't know if that helps narrow down the problem any.

This seems to be a crash in teh windows directx but that shouldn't happen in the Firefox safemode.
Can you try to upgrade your graphic card driver ?

Thanks for the suggestion -- I love Firefox and greatly appreciate you and everyone else who's worked hard to make it the browser that it is. Unfortunately, updating my graphics driver did not resolve the problem. More on that, and some new information, below:
1. This bug no longer occurs when Firefox is started in safemode, for me at least. I don't know when this changed since I have not tried loading in safemode since I first tried resolving the problem back around ~beta 8ish.
2. Rapidly changing the page from the home page before Firefox dies does not help, at least not consistently. Further experimenting has yielded several crashes despite performing this step in time.
3. This issue did not occur in Firefox 3 on the same system. I don't remember the exact version, but it was up to date at least as early as mid-February. This issue did not occur in the first Firefox 4 beta I downloaded (maybe 5-6, I'm sorry I don't remember exactly), but it did get introduced with an update very shortly thereafter.
4. I upgraded my graphics driver to the latest available nVidia version, which was 270.51 (beta). My previous driver was 267.24 (beta) installed 3/9/11. I have searched the nVidia forums and do not see any issues with their drivers and Firefox that are similar to the one at hand here. (There are a few with Youtube/flash videos and 3d). I have had no issues with the nVidia driver and any other program, but if you think the problem is related to the video drivers, I can try a complete removal of the beta drivers and replace them with the latest stable/recommended release (266.58). It may be a bit of an undertaking, though, not sure how well nVidia drivers downgrade.

UPDATE: My evening plans fell through so I went ahead and tried downgrading the video driver to the latest stable/recommended release. This did not resolve the problem, but it does appear to be noticeably less frequent than before. It now only crashes 2-3 times before it will load and stay up successfully.
My system was reporting DX 9.0c, which is supposedly the latest version of DX, but I went ahead and ran the MS DX webinstaller, which downloaded and installed some new stuff. Version number is still reported as 9.0c, however. I did not see any noticeable difference in crash frequency after this step. (All Windows Update updates were already installed).
I've reviewed all processes in the task manager and my startup profiles and disabled or removed any other application or utility that was graphics-related. Again, I did not see any noticeable difference from this either.

In that case disable the hardware acceleration in tools/options/advanced/general.
That should stop the crashes.

I am having this problem as well with FF 5.0 I have had to disable all most all plugins and addons to get FF to open and not crash.

I also checked my settings for my video cards and they are set to full hardware acceleration. I am running 2 nVidia GTX 295s with each core running separately.

You will also notice from the graphic section post that I am running the most recent and stable video driver that is 275.33.

I have tried all the various things listed above some worked for a while but I am back to square one, and at this point I sometimes have to use the other browser(IE) to get online. So things are getting worse. I never had this problem till I upgraded to FF5.

I thought I had the problem resolved, but it is back. I just updated my drivers and it is now doing the same thing again. I click on the browser icon to open it and it will open for about 1 - 2 seconds then crashes this will happen between 10 and 20 times before it will stay open.

The crash is inside the windows directx dll
The safemode should help as it disables hardware acceleration and you can disable the hwa in the Firefox options and there is not much we can do
