665128 - Kuma: Default edit toolbar changes

We need some changes to the default toolbar's available options. This is the first of what will likely be a series of bugs on the matter.

First, we need to add as top-level buttons:

H1-H5 (or at least H3 if there's not room for them all).
<code>
<pre>
<dl>, <dt>, and <dd>

We can get rid of the font popup, since we don't actually want people changing fonts except under exceptional circumstances (that is, when demonstrating HTML features, in which case that would be a code sample).

Also, get rid of:

Size
All of the form buttons

I would prefer to have them as quick to access buttons instead of in a drop-down but if that's a problem we can probably live without it.

raymond, can you check this again now that it's been a couple days. if it was a cache issue I would think the cache is clear by now.

also, did you file a bug for "can't preview more than once" issue?

It seems this has been resolved. We can reopen this if we begin seeing issues again.
