669921 - duplicate collections could be created in Add to Collection->Start a new collection

Steps:
--------
1) Log in to https://addons.allizom.org
2) Load add-on details page: https://addons.allizom.org/en-US/firefox/addon/fireftp/
3) Click on Add to Collection:Start a new collection and create a collection with name "test123"
4) Click again on "Start a new collection" and create another collection with same name "test123"

Expected: An error message indicating duplicate collection

Observed behavior: Duplicate collection is created

Note: After step#3, click on some other part in details page to close "Add to collection" box and then trying to create a duplicate collection shows the expected error message "This url is already in use by another collection"

Thanks for filing this.  Due to resource constraints we are closing bugs which we won't realistically be able to fix.  If you have a patch that applies to this bug please reopen.
