Allow a site administrator to look up user IP address of a banned user and ban new accounts from that IP for the duration of the ban. This does not work on MindTouch because everything comes from the netscaler, so we need to work out how to get the correct IP before doing the bans.

We actually don't want banning by IP to be automatically applied; we want it to be done manually. This does mean we need to keep track of the IP addresses used by users, so we can click a "ban this user's IP" button.
