I haven't been able to come up with good reliable reproduction steps for this, but the problem happens constantly.
I have about 8-10 tab groups each with 5-40 tabs in them. Routinely when I cmd-w to close my current tab, firefox will jump me to some entirely different tab group and focus a random tab there instead of focusing the tab directly next to the one I closed.

Do you have any add-ons installed that may interfere here? Bug 688097 reports that All in One gesture add-on caused similar problems.
Could you also specify the Firefox version you are using?

v9.0.1. I do also have All in One installed, Looks like these exact steps from Bug 688097 trigger the bug, so most likely I just duped it. I'll disable that extension and make sure before I close this.

STR (TG = tab group):
1. Launch Panorama, Create empty TG1, TG2.
2. Open TG1 and open tab1(URL1) as the new tab.
3. With Panorama, switch to TG2 and open tab2(URL2) as the new tab.
4. With Panorama, switch back to TG1.
5. Open new empty tab3 and tab4. Close tab4 and tab3. After closing tab3, you will be switched from TG2 to TG1.
